You're probably *not* being ego attacked.  The message you see, whether it's
the "stabbing pain", the "black out", the "vision clouds", or something
similar, is a standard message which can probably be translated into
"someone attempted to cast a spell on you which failed to overcome your MR".
The most likely action was that someone tried to scry you.  This kind of
thing tends to happen if you enter a major city where other PC's are hanging
out, if you've recently gotten into a tussle with someone, or if you're
about to get into a tussle with someone.  Generally speaking, it's *not*
a good sign, but it might not be the threat that *I* make it out to be -
I tend to be somewhat paranoid.

As for MR, well, it depends.  The Overlord in Realm has a MR of 100 or so.
My PC in Realm 5 had a MR of 20 or so by the time I stopped playing.  MR
varies, because it's completely dependent on what items the character has
and what status (ignoring shells, which are transient).
