Dunno what stat changes you get (rumor has it your spirit affinity goes up),
but an INVESTIGATE of the title shows this :

Description:

Dark Friends are servants of evil.  They have sworn to help the king of
the dead, Neclor, establish his rule over all peoples in the Empire,
Wilds, Fringe, and Fold.  Cleansing the world of the false Elemental
Gods and all their priestly followers is also one of their charters.
It is unclear how a character becomes a Dark Friend - they are a
secretive group.

Also, it looks as if the next title in the chain is t6667, Nightstalker (I've
seen some NPC's with that title).
