There are indeed some new structures available now (in any case the
UK-version) has it, and some of the older bugs are repaired.For so far I
know all of the 'former' high level structures are open, exceptfor the Tomb
of Mummydon also added are some new creatures and items. And KJC claims that
there are some new spells. But better is the fact that they are working on
the game again, something that wasn't the case at ABM.

Sorry but I know nothing about the service down there at the States. I
could say just look into your old files and look if there is a nice
Tainted Hallow, Sunken Graveyard and so on arround. I think that you can
best go to the nearest Sunken Graveyard if you want to have a realfight!
